In the first scenario, only the women in extreme categories were reassigned: women in the low SES category were assigned to middle SES category (20–p80) and women aged < 18 years or > 35 years were assigned to the reference category ‘25–29 years’. In the second scenario, all women were ‘assigned’ to the most favourable values: highest SES category (>p80), multiparous, western ethnicity, and 25–29 years old.
